Operation note templates

A small library of interactive form-builders for personal operative documentation. Each template emits a Markdown note structured to RCS Good Surgical Practice 2014 and MCNZ standards. No patient data is stored or transmitted; everything runs in your browser.

Templates only — verify before pasting. No patient data is stored or transmitted. You remain the author of, and responsible for, the operation note.

Skin and soft tissue

Excisions, biopsies, and reconstructions of cutaneous and subcutaneous lesions — benign and malignant.

  1. Operation note v1.1

    Skin lesion excision

Excision of one or more benign or malignant cutaneous lesions on a single patient. Closure morphs per lesion: direct / FTSG / STSG / local flap.

    Indication · Per-lesion pathology and margin · Per-lesion closure procedure · Per-lesion specimen orientation · Shared post-op plan

  2. Operation note v1.0

Melanoma wide local excision + SLNB

Wide local excision of biopsy-proven cutaneous melanoma with sentinel lymph node biopsy. Margin matrix per ANZ Melanoma Guidelines (Sladden 2018) / SCNZ 4th Edition.

Histology · AJCC 8 pT · Configurable margin matrix · Lymphoscintigraphy · Patent blue + gamma probe · WLE · Surveillance plan

Hand surgery

Acute hand surgery: trauma (laceration / fracture / tendon / nerve, one accident covered by one note) and infection (PFT / deep space / septic arthritis). Almost always ACC-funded for trauma.

  1. Operation note v2.0

    Hand trauma

One trauma event, multiple structures damaged. Check the boxes for the structures injured (laceration / fracture / flexor / extensor / nerve); the output composes a single coherent note in anatomic order with a most-restrictive-wins post-op plan.

Mechanism · Per-structure findings + repair steps · Splint chosen by deepest repair · Hand therapy timeline · ACC claim

  2. Operation note v1.0

    Hand infection drainage

Surgical drainage of hand-space infection. Type toggle: pyogenic flexor tenosynovitis / deep palmar space / septic arthritis.

    Diagnosis · Type-specific approach · Cultures · Lavage · Splint · IV antibiotic plan · Re-look plan

Free flap reconstruction

Microvascular tissue transfer for complex composite defects after trauma or oncologic resection.

  1. Operation note v1.0

    Free flap reconstruction

    Microvascular tissue transfer. Modular: recipient prep + flap-type-specific harvest + anastomosis + inset + post-op flap-monitoring plan. Flap library: ALT / RFFF / fibula / DIEP / LD / gracilis / MSAP / SCIP.

    Defect · Recipient vessels · Harvest module · Coupler + 9-0 anastomosis · Cook–Swartz Doppler · HDU monitoring schedule
