Non-vascularised bone graft remains the appropriate choice for routine scaphoid waist non-union without significant deformity or AVN. Cancellous graft is used when scaphoid length is preserved; structural corticocancellous wedge graft (Russe, Fisk-Fernandez) is used to correct humpback deformity. Union rates are equivalent to vascularised graft in this subset.
