Avascular necrosis of the scaphoid is reported in 13–40% of proximal pole fractures and approaches 100% in fractures isolating the proximal fifth. Diagnosis combines MRI signal change with intra- operative confirmation of absent punctate bleeding from the proximal fragment after debridement. AVN influences the choice between vascularised and non-vascularised reconstruction.
