Anatomical snuffbox tenderness is the classical clinical sign of scaphoid injury. Sensitivity is high (above 90%) but specificity is modest. Combined with scaphoid tubercle tenderness and pain on axial thumb compression, the predictive value for true scaphoid fracture rises substantially. Snuffbox pain on ulnar deviation within 72 hours of injury is the most discriminating individual finding.
