DISI is the lunate component of the carpal collapse pattern that follows scaphoid fracture, scaphoid non-union, or scapholunate dissociation. On lateral radiograph, the radiolunate angle exceeds 15 degrees and the scapholunate angle exceeds 60 degrees. DISI is the radiographic signature that prompts structural reconstruction with anterior wedge graft to restore scaphoid length and lunate alignment.
