This page provides the information required of a regulated medical practitioner operating a professional website addressed to the public, in accordance with the Medical Council of New Zealand (MCNZ) Statement on Advertising, the Polish Ustawa o działalności leczniczej (Act on Medical Activity), Article 71 of the Kodeks Etyki Lekarskiej (KEL — Code of Medical Ethics, in force 1 January 2025), and the disclosure expectations of Regulation (EU) 2016/679 (the General Data Protection Regulation, GDPR).

The Polish title lekarz corresponds to the English “physician” (medical doctor); the abbreviation lek. may appear on Polish-language documents in place of “MD”. FEBOPRAS is the Fellow of the European Board of Plastic, Reconstructive and Aesthetic Surgery diploma issued by the Union Européenne des Médecins Spécialistes (UEMS); FEBHS is the European Board diploma in Hand Surgery issued by the Federation of European Societies for Surgery of the Hand (FESSH).

Professional title and qualifications recognition

The professional title lekarz is a regulated profession within the meaning of Directive 2005/36/EC. Specialist qualifications were obtained in Poland and supplemented by European Board diplomas (FEBOPRAS; FEBHS / FESSH Diploma in Hand Surgery). Recognition of qualifications for the purposes of practice in New Zealand is held with the Medical Council of New Zealand under registration 93463.

Professional indemnity

Professional liability cover applicable to the current New Zealand practice is maintained as required by the Accident Compensation Act 2001 framework and through medical indemnity arrangements with a recognised medical defence organisation. Cover applicable to clinical practice in Poland will be maintained in accordance with Article 18 of the Ustawa z dnia 15 kwietnia 2011 r. o działalności leczniczej and the implementing regulation on compulsory liability insurance (ubezpieczenie odpowiedzialności cywilnej) for entities carrying on medical activity.

Tax identification

No Polish Numer Identyfikacji Podatkowej (NIP — Tax Identification Number) is currently held in connection with the operation of this website, as no commercial medical activity is presently carried on within the territory of the Republic of Poland. Tax identification will be added to this imprint at the point at which Polish practice is registered.

Nature of website content

This website provides information about Dr Gładysz, his training, his clinical interests and his published work. It does not constitute medical advice, does not establish a doctor–patient relationship, and does not advertise medicinal products within the meaning of section 58(1) of the Medicines Act 1981 (New Zealand) or the Polish Prawo farmaceutyczne. References to medical devices comply with the disclosure requirements of the Ustawa z dnia 7 kwietnia 2022 r. o wyrobach medycznych and the implementing regulation on advertising of medical devices in force since 1 January 2023.

Information about services (informacja o oferowanych usługach) presented on this site is provided in accordance with Article 71 of the KEL and within the limits set by Article 14 of the Ustawa o działalności leczniczej, which preserves the prohibition on advertising of medical activity in Poland.
